Safe and Quality Treatment

You have the right to receive competent care that meets accepted clinical and safety standards, delivered in a clean, well-maintained, and properly equipped facility.
Information and Understanding

You have the right to clear, honest, and understandable information about your diagnosis, treatment plan, and prognosis. You may ask questions at any time and expect answers in a language or manner you understand.
Informed Consent

You have the right to be informed of the benefits, risks, and alternatives of any treatment or procedure before giving your consent, and to participate actively in decisions about your care.
Refusal of Treatment

You have the right to refuse any treatment or procedure to the extent permitted by law, and to be informed of the medical consequences of your decision.
Privacy and Confidentiality

You have the right to personal privacy during treatment and to the confidentiality of your medical records and health information, consistent with the Data Privacy Act and applicable laws.
Continuity of Care

You have the right to reasonable continuity of care, to be informed of your ongoing treatment schedule, and to receive appropriate instructions for care after each session and upon discharge or referral.
Access to Your Health Information

You have the right to access and request an explanation of the information in your medical record, subject to facility policy and law.
Voice Concerns and Grievances

You have the right to express concerns, complaints, or suggestions without fear of discrimination or reprisal, and to expect a fair and timely response.
Know Your Care Team

You have the right to know the identity and professional role of the physicians, nurses, and staff involved in your care.

Your Responsibilities as a Patient

To help us provide the safest and most effective care, we ask that you:

Provide Accurate Information

Share complete and honest information about your medical history, current medications, allergies, and any changes in your condition.
Follow the Agreed Plan of Care

Adhere to your prescribed dialysis schedule, treatment plan, medications, diet, and fluid restrictions, and inform your care team if you are unable to do so.
Attend Scheduled Sessions

Arrive on time for your dialysis sessions and notify the center in advance if you need to reschedule or will be absent.
Ask Questions

Speak up if you do not understand any instruction, information, or aspect of your care.
Show Respect and Consideration

Treat staff, other patients, relatives, and visitors with courtesy. Help maintain a calm, safe, and orderly environment, and be considerate of shared spaces and belongings.
Observe Facility Policies

Follow the center's rules, including those on safety, infection control, visiting, and conduct within the facility.
Meet Financial Obligations

Fulfill the financial responsibilities associated with your care in a timely manner, and coordinate with our staff regarding billing, PhilHealth, or other coverage.
Take Care of Your Access and Health

Follow instructions for the care of your vascular access, report any problems promptly, and take an active role in maintaining your overall health.
Provide Feedback

Share your experiences, concerns, and suggestions so we can continue to improve our services.
